Non-Invasive Liver Stiffness Assessment

What does FibroScan® measure?

FibroScan® uses transient elastography to estimate liver stiffness non-invasively. Increased liver stiffness may be associated with fibrosis, although results must always be interpreted in clinical context.

Liver stiffness

Measurements are reported in kilopascals (kPa) and can contribute to non-invasive assessment of liver fibrosis.

Interpretation depends on context

The meaning of a liver-stiffness result depends on the underlying liver condition, test quality and other clinical factors. A single universal kPa threshold should not be applied to every patient.

Non-invasive

FibroScan® does not use ionising radiation and does not require a needle or biopsy to obtain a stiffness measurement.

Important clinical context: FibroScan® is not interpreted in isolation. Liver stiffness can be influenced by factors other than fibrosis, and appropriate interpretation depends on the suspected liver condition, clinical history, blood tests and imaging. NICE recommends discussing the accuracy and limitations of non-invasive tests and ensuring that professionals performing or interpreting them are appropriately trained.
Common Liver Findings

Findings that may be identified during liver assessment

Not every abnormality can be diagnosed by ultrasound or elastography alone. Findings are interpreted in clinical context and additional medical assessment may be required.

MASLD

Fatty liver / hepatic steatosis

Ultrasound may demonstrate features of steatosis. Metabolic dysfunction-associated steatotic liver disease (MASLD) is the current terminology used when steatotic liver disease occurs in association with cardiometabolic risk factors.

FIB

Increased liver stiffness

FibroScan® may demonstrate increased liver stiffness, which can contribute to assessment for clinically significant fibrosis.

CYS

Liver cysts

Simple hepatic cysts are common and can usually be characterised sonographically.

LES

Focal liver lesions

Ultrasound may detect focal liver abnormalities. Indeterminate or suspicious lesions may need contrast-enhanced imaging, CT, MRI or specialist review.

CIR

Chronic liver changes

Coarse liver texture, nodular contour, altered morphology or other features may raise concern for chronic liver disease.

GAL

Gallstones & biliary findings

Depending on examination coverage, gallstones and biliary dilatation may also be identified.

Fatty Liver & MASLD

Fatty liver is not just an ultrasound finding

Fat accumulation within the liver may occur in association with excess weight, type 2 diabetes, abnormal blood lipids, metabolic syndrome, alcohol exposure, medications and other conditions.

The modern term metabolic dysfunction-associated steatotic liver disease (MASLD) has replaced the older term NAFLD in international hepatology guidance.

Ultrasound can demonstrate visible steatosis, while quantitative attenuation techniques such as UGAP can provide additional information about liver fat. Fibrosis risk is a separate question, which is why liver stiffness assessment and appropriate clinical risk stratification can be important.

Clinical Context

Who may benefit from discussing liver assessment?

Liver tests are most useful when they answer a clinically relevant question rather than being interpreted as isolated numbers.

Abnormal liver blood tests

Raised ALT, AST, ALP, GGT or bilirubin may lead to liver imaging or further medical investigation.

Suspected fatty liver

Liver assessment may be appropriate where fatty liver has been suspected clinically or identified previously.

Type 2 diabetes or metabolic risk

Diabetes, obesity and other cardiometabolic risk factors are associated with steatotic liver disease.

Alcohol-related liver risk

Harmful or prolonged alcohol exposure can increase the risk of fibrosis and alcohol-related liver disease.

Known chronic liver disease

Patients with established liver disease may require imaging or non-invasive fibrosis assessment according to their medical pathway.

Repeat imaging may be appropriate when a liver abnormality has previously been identified and follow-up recommended.

Important Clinical Information

What liver ultrasound and blood tests cannot tell you alone

A clinically responsible liver-health assessment recognises the limitations of each individual test.

Normal blood tests do not exclude cirrhosis

Routine liver blood tests can be normal in patients with significant chronic liver disease. NICE specifically advises against using routine liver blood tests to rule out cirrhosis.

Ultrasound does not stage fibrosis reliably

Conventional ultrasound can show morphology and signs of chronic liver disease, but early fibrosis may not be apparent. Elastography addresses a different question by measuring liver stiffness.

Abnormal results may require hepatology review

Significant stiffness elevation, suspicious focal lesions, signs of cirrhosis or other concerning findings may require GP, gastroenterology or hepatology referral and additional testing.

Seek urgent medical care rather than routine private scanning if you develop severe illness, confusion, vomiting blood, black stools, rapidly increasing abdominal swelling, severe jaundice, collapse or other acute concerning symptoms.

Preparation

For combined liver ultrasound and FibroScan® appointments, follow the fasting instructions in your booking confirmation. UKSONO’s current liver-health information advises fasting for approximately 6–8 hours, with water permitted.

Diagnostic ultrasound

Gel is applied to the upper abdomen and the liver is examined using diagnostic ultrasound.

FibroScan® / UGAP

Where included, liver stiffness and/or quantitative liver-fat measurements are obtained using the relevant technique.

Bloods & results

Blood samples are arranged where included. Scan findings are normally available the same day, with laboratory results following separately.

How much is a private liver scan?
UKSONO Healthcare’s focused private liver ultrasound costs £185. More comprehensive liver packages range from £300 to £700 depending on whether FibroScan®, UGAP, blood tests and GP consultation are included.
What is the difference between liver ultrasound and FibroScan®?
Diagnostic ultrasound primarily evaluates liver structure and visible abnormalities. FibroScan® uses transient elastography to measure liver stiffness, providing different information relevant to fibrosis assessment. They are complementary rather than interchangeable tests.
What does FibroScan® measure?
FibroScan® measures liver stiffness using transient elastography. Results are reported in kilopascals and interpreted according to the clinical context, underlying liver condition and test quality.
Does a high FibroScan® result definitely mean cirrhosis?
Not automatically. Higher liver stiffness can be associated with fibrosis and cirrhosis, but liver stiffness may also be affected by other clinical factors. Results should therefore be interpreted with the patient’s history, other investigations and the relevant disease-specific thresholds.
What is UGAP?
UGAP stands for Ultrasound-Guided Attenuation Parameter. It measures the attenuation of ultrasound through liver tissue and can provide quantitative information relevant to hepatic fat or steatosis assessment.
Can ultrasound detect fatty liver?
Yes. Conventional ultrasound can demonstrate sonographic features of hepatic steatosis, particularly when fat accumulation is sufficiently marked. Quantitative techniques such as UGAP can provide additional information.
What is MASLD?
MASLD means metabolic dysfunction-associated steatotic liver disease. It is the contemporary terminology for steatotic liver disease associated with cardiometabolic risk factors and replaces the older term NAFLD in current international hepatology guidance.
Can liver ultrasound diagnose liver fibrosis?
Conventional ultrasound can show morphological features of advanced chronic liver disease, but it does not reliably stage early fibrosis. Elastography, blood-based fibrosis tests and other clinical pathways are used for fibrosis risk assessment.
Can normal liver blood tests rule out liver disease?
No. Normal routine liver blood tests do not reliably exclude significant fibrosis or cirrhosis. NICE specifically advises not to use routine laboratory liver blood tests to rule out cirrhosis.
Can ultrasound detect cirrhosis?
Ultrasound may show features suggestive of advanced chronic liver disease or cirrhosis, such as abnormal liver morphology, coarse echotexture, nodular contour, splenomegaly or ascites. However, cirrhosis assessment may require elastography, laboratory tests and specialist review.
Can a liver scan detect liver cancer?
Ultrasound can identify some focal liver abnormalities, but a routine private liver scan is not a substitute for a formal hepatocellular carcinoma surveillance programme in patients at risk. Any suspicious focal lesion may require specialist assessment and further imaging such as contrast-enhanced CT or MRI.
Do I need to fast?
For liver ultrasound and combined ultrasound/FibroScan® appointments, follow the fasting instructions provided with your booking. UKSONO’s current liver-health information advises approximately 6–8 hours fasting, with water permitted.
Do I need a GP referral?
No. Suitable patients can self-refer and book a private liver scan or liver-health package directly.
Will I receive my liver scan results the same day?
The main ultrasound and elastography findings are normally discussed during or immediately after your appointment, with a formal scan report provided. Laboratory results take additional processing time where blood tests are included.
How quickly do blood-test results return?
UKSONO’s current liver-health packages generally expect laboratory results within approximately 24–48 hours, although laboratory turnaround times can vary.
What blood tests are included in the Premium package?
The Premium package includes a standard liver-function blood-test panel covering markers such as ALT, AST, ALP, GGT, bilirubin and proteins according to the package specification.
What does the Ultimate Liver Health Check include?
The £700 Ultimate package includes diagnostic liver ultrasound, FibroScan®, an advanced liver-function blood-test panel and a 15-minute online GP consultation to discuss results and appropriate next steps.
What happens if significant liver fibrosis is suspected?
Your report should recommend appropriate clinical follow-up. Depending on the findings, this may include GP review, repeat testing, gastroenterology or hepatology referral, additional blood tests, CT, MRI or another investigation.
Is FibroScan® painful?
FibroScan® is non-invasive and normally well tolerated. A probe is positioned over the right upper abdomen and a series of measurements are obtained.
Does liver ultrasound or FibroScan® use radiation?
No. Diagnostic ultrasound and transient elastography do not use ionising radiation.
Which liver-health package should I choose?
The most appropriate option depends on the clinical question. A liver ultrasound evaluates structural appearance; FibroScan® focuses on liver stiffness; UGAP provides quantitative liver-fat information; and blood-test packages add biochemical information. Patients already under medical care should follow the investigation pathway recommended by their clinician where possible.
