Plantar fascia ultrasound provides high-resolution assessment of fascial thickness, echotexture, fibre continuity and insertional changes at the heel.
It is particularly useful for morning first-step pain, persistent plantar heel pain, suspected plantar fasciitis and suspected partial tearing.
Adjacent heel soft tissues can also be assessed where symptoms or swelling extend beyond the fascia itself.

The examination is focused on the plantar fascia and symptomatic heel region.
Fascial thickness
Measurement of the plantar fascia, particularly at its calcaneal insertion.
Plantar fasciitis features
Assessment for thickening, hypoechoic change and loss of the normal fibrillar appearance.
Partial tears
Assessment for focal fibre disruption, defects and surrounding soft-tissue change.
Insertional changes
Assessment of the plantar fascia at its calcaneal attachment and adjacent superficial tissues.
Adjacent soft tissues

Common reasons for plantar fascia ultrasound
Morning first-step pain
Heel pain that is particularly noticeable on the first few steps after rest.
Persistent plantar heel pain
Ongoing pain at the underside of the heel despite rest or conservative measures.
Suspected plantar fasciitis
Symptoms and tenderness suggesting inflammatory or degenerative change of the plantar fascia.
Suspected tear
Sudden or focal heel pain where partial fascial injury is suspected.
Sports or overuse symptoms
Heel pain associated with running, prolonged standing or repetitive loading.
Focal swelling
Localised swelling or tenderness around the plantar heel requiring targeted assessment.
