Elbow ultrasound provides high-resolution assessment of the common extensor and flexor tendons, distal biceps, triceps, olecranon bursa, selected ligament regions, joint fluid and the ulnar nerve.
It is particularly useful for suspected tennis elbow, golfer’s elbow, tendon injury, bursitis, focal swelling and selected nerve-related symptoms.
Ultrasound can also be performed dynamically while the elbow bends or straightens when movement-related symptoms need assessment.
Structures assessed during an elbow ultrasound
The examination is tailored to the location of pain, injury history and clinical question.
Common extensor tendon
Assessment for tennis-elbow-type tendinopathy, partial tearing, calcification and associated hyperaemia where present.
Common flexor tendon
Assessment for golfer’s-elbow-type tendinopathy, tearing and insertional change.
Distal biceps tendon
Assessed for tendinopathy, partial or complete tearing, fluid and insertional abnormality where visible.
Triceps tendon
Assessment of the distal triceps insertion for tendinopathy, tearing and calcific change.
Olecranon bursa & joint fluid
Assessment for bursitis, superficial fluid collections and visible elbow-joint effusion.
Ulnar nerve & ligament regions
The ulnar nerve at the cubital tunnel and relevant collateral-ligament regions can be assessed, including dynamically where appropriate.

Tennis elbow symptoms
Lateral elbow pain, tenderness and pain related to gripping or repetitive wrist extension.
Golfer’s elbow symptoms
Medial elbow pain related to flexor-pronator loading or repetitive activity.
Tendon injury
Suspected distal biceps, triceps or common tendon injury following trauma or repetitive loading.
Olecranon swelling
Swelling at the back of the elbow where bursitis or a superficial fluid collection is suspected.
Ulnar nerve symptoms
Selected symptoms around the cubital tunnel where an ulnar-nerve abnormality is clinically suspected.
