Detailed fetal anatomy

A systematic head-to-toe fetal assessment.

The anatomical component follows a structured mid-pregnancy approach. Each region is examined in the relevant ultrasound planes where technically obtainable, with limitations documented clearly.

01

Brain & head

Skull shape, intracranial anatomy, ventricles, posterior fossa and key midline structures.

02

Face & lips

Facial profile and lips where fetal position allows appropriate views.

03

Spine

Longitudinal and transverse assessment including the sacral region where visible.

04

Heart

Situs, four chambers, outflow tracts and three-vessel/trachea relationships.

05

Chest & diaphragm

Thoracic anatomy and diaphragm integrity within ultrasound limitations.

06

Abdomen

Abdominal situs, stomach region, abdominal wall and key abdominal structures.

07

Kidneys & bladder

Both renal areas and the fetal bladder where technically visible.

08

Skeleton & limbs

Long bones, limb presence and overall skeletal development.

Fetal cardiac protocol

A dedicated look at the fetal heart.

The heart is assessed systematically because major congenital cardiac abnormalities can be subtle. Core screening views are obtained where technically possible.

01
Situs & lateralityOrientation of the fetal heart and abdominal organs.
02
Four-chamber viewAssessment of cardiac chambers, symmetry and septal region.
03
Left ventricular outflow tractAortic outflow assessed in relation to the left ventricle.
04
Right ventricular outflow / 3-vessel viewPulmonary outflow and great-vessel relationships.
05
3-vessel & trachea viewGreat vessels and their relationship to the trachea.
Uterine artery Doppler

Assessing maternal blood flow to the placenta.

The uterine arteries are maternal vessels that carry blood towards the uterus and placenta. Doppler analysis measures the resistance pattern within these vessels and provides additional information about placental circulation.

Bilateral assessmentBoth left and right uterine artery waveforms are assessed where technically obtainable.
Pulsatility Index (PI)PI reflects resistance within the sampled vascular waveform and is interpreted against gestation.
Mean uterine artery PIThe left and right measurements can be combined into a mean PI where both sides are successfully obtained.
Placental-risk contextRaised resistance can be associated with a greater chance of placental dysfunction, fetal growth restriction and pre-eclampsia.
Timing

Why this combination is centred around mid-pregnancy.

The two components have overlapping but not identical timing. The combined appointment is designed around the period when a detailed anatomy survey and uterine artery Doppler can both be clinically useful.

Fetal anatomy

18+0 to 20+6 weeks

This is the current NHS FASP recommended window for the 20-week screening scan.

Uterine artery Doppler

Usually around 20–24 weeks

Second-trimester uterine artery Doppler is commonly used during this period to assess placental vascular resistance.

Combined appointment

Around 20 weeks

Booking around the 20-week period offers a practical overlap for detailed fetal anatomy and uterine artery Doppler assessment.

Preparation

Come prepared for a detailed examination.

This is a comprehensive appointment and image acquisition depends on fetal position, maternal factors and the ability to obtain reliable Doppler waveforms.

Bring your pregnancy datesKnow your EDD and bring relevant previous ultrasound or maternity information if available.
Wear comfortable clothingChoose clothing that allows easy access to the abdomen.
Allow for fetal positionSome anatomical views can take longer if the baby is positioned unfavourably.
Continue NHS screeningThis private assessment does not replace the NHS FASP screening pathway or your routine maternity care.
01
Clinical history & gestationYour dates, relevant risk factors and reason for booking are reviewed.
02
Detailed fetal anatomyMajor fetal structures are assessed systematically in appropriate ultrasound planes.
03
Fetal cardiac viewsCore screening cardiac views are obtained where technically possible.
04
Uterine artery DopplerLeft and right uterine artery waveforms are sampled and PI measurements recorded where obtainable.
05
Results & onward guidanceBoth the anatomical and Doppler findings are explained, with appropriate NHS or specialist signposting if needed.
Interpretation & limitations

Powerful information, interpreted with clinical context.

This combined scan adds placental-flow information to a detailed structural fetal assessment, but neither component can predict or exclude every pregnancy complication.

What the combined scan gives you

A detailed mid-pregnancy anatomical survey plus bilateral uterine artery Doppler, providing information about fetal structure and maternal placental vascular resistance in one appointment.

What it does not replace

Your NHS 20-week screening scan, formal pre-eclampsia risk assessment, maternal blood-pressure surveillance, specialist fetal medicine review, later growth surveillance, umbilical artery Doppler or maternity triage when clinically indicated.

Results & reporting

One report bringing both assessments together.

Your Sonographer explains the fetal anatomical findings and uterine artery Doppler results during the appointment. The report documents anatomical structures assessed, recognised limitations and Doppler measurements where obtained.

Reassuring findingsNormal appearances are documented in relation to gestation and the limitations of ultrasound.
Incomplete anatomy viewsIf fetal position prevents complete assessment, this is documented and the appropriate next step is explained.
Raised uterine artery resistanceThe Doppler finding is explained in context, with advice to discuss it with your maternity team where additional surveillance may be appropriate.
Unexpected structural findingSuspected abnormalities are discussed sensitively and signposted for appropriate NHS maternity or fetal medicine assessment.
!
This private scan is not an emergency maternity assessment.

Heavy bleeding, severe or worsening pain, leaking fluid, contractions when not expected, fainting, severe headache or visual symptoms, fever, feeling very unwell or concerns about fetal movements later in pregnancy require appropriate NHS maternity assessment.

Frequently asked questions

Detailed answers before you book.

Understand timing, anatomy scope, uterine artery Doppler, placental resistance, pre-eclampsia limitations, reporting and when NHS review remains essential.

What is the Fetal Anatomy & Uterine Artery Doppler Scan?
This is a combined private mid-pregnancy ultrasound that brings together a detailed fetal anatomical assessment with Doppler evaluation of the maternal uterine arteries. The anatomy component is structured with reference to the NHS Fetal Anomaly Screening Programme 20-week scan framework, while the uterine artery Doppler assesses resistance to blood flow in the maternal vessels supplying the placenta.
How much is the combined scan?
The Fetal Anatomy & Uterine Artery Doppler Scan is £250 at Tunbridge Wells and Orpington and £300 at Canary Wharf.
When is the best time to book?
The detailed anatomy component is aligned with the NHS FASP mid-pregnancy screening window of 18 weeks plus 0 days to 20 weeks plus 6 days. Uterine artery Doppler is commonly assessed around 20 to 24 weeks. For a combined appointment, booking around the 20-week period is generally the most practical.
Is this the same as the NHS 20-week screening scan?
No. UKSONO provides a private detailed ultrasound assessment structured with reference to the current FASP anatomy and fetal cardiac framework. It is not part of the nationally commissioned NHS FASP screening programme and does not replace your NHS screening appointment.
What fetal anatomy is examined?
The scan systematically reviews major fetal structures including the brain and head, face, spine, heart and outflow views, chest, abdomen, kidneys, bladder, abdominal wall, skeleton and limbs, together with standard fetal biometry where technically obtainable.
What heart views are included?
Where technically obtainable, the cardiac review includes fetal situs, four-chamber view, left ventricular outflow tract, right ventricular outflow tract or three-vessel view, and the three-vessel-and-trachea view.
What is a uterine artery Doppler?
Uterine artery Doppler uses colour and pulsed-wave Doppler to assess blood-flow waveforms in the left and right uterine arteries. The pulsatility index can be measured and interpreted in relation to gestational age.
What does uterine artery Doppler tell us?
Uterine artery Doppler provides information about resistance within the maternal circulation supplying the placenta. Higher resistance can be associated with a greater chance of placental dysfunction, fetal growth restriction and pre-eclampsia, but the Doppler result is not a diagnosis on its own.
Does this scan diagnose pre-eclampsia?
No. Pre-eclampsia is not diagnosed by uterine artery Doppler alone. Doppler findings should be interpreted alongside maternal history, blood pressure, symptoms and, where relevant, other clinical or biochemical assessments.
Does the scan check the placenta and amniotic fluid?
Yes. Placental position and an amniotic fluid overview are reviewed as part of good clinical obstetric ultrasound practice. The uterine artery Doppler assesses maternal blood flow supplying the placenta rather than fetal umbilical cord blood flow.
Is uterine artery Doppler the same as umbilical artery Doppler?
No. Uterine artery Doppler assesses maternal vessels supplying the placenta. Umbilical artery Doppler assesses blood flow in the baby's umbilical cord and is usually used in later-pregnancy growth and wellbeing assessment.
What if an anatomical abnormality is suspected?
Any unexpected or suspected structural finding will be explained sensitively and documented. Where appropriate, you will be advised to contact your NHS maternity team or fetal medicine service for specialist assessment.
What if the uterine artery Doppler is raised?
The Sonographer will explain the Doppler finding and document it in your report. Depending on the result and your clinical context, you may be advised to discuss the finding with your midwife, obstetric team or fetal medicine service, as additional growth surveillance may be appropriate.
Will I receive a report and images?
Yes. You receive a same-day digital report and ultrasound images. The report documents both the fetal anatomical assessment and the uterine artery Doppler findings, including Doppler indices where obtained.
Do I need a referral?
No. You can self-refer and book directly. Please continue to attend all NHS antenatal and screening appointments, including your NHS 20-week screening scan if offered.
