Pre-IVF Baseline Pelvic Ultrasound with Antral Follicle Count
A detailed baseline transvaginal ultrasound before IVF or assisted conception, assessing the uterus, endometrial thickness and appearance, both ovaries, ovarian size and antral follicle count in each ovary. The report is designed to provide your fertility team with clear baseline ultrasound measurements before treatment.
Important: this baseline service requires a transvaginal ultrasound because ovarian detail, antral follicle counting and endometrial measurement require high-resolution pelvic imaging. The examination is only performed after explanation and explicit consent, and you can ask for the examination to stop at any time.

A pre-treatment snapshot of the uterus, endometrium and ovaries
A pre-IVF baseline scan provides ultrasound measurements before ovarian stimulation or another assisted-conception treatment cycle. It gives the fertility team a structural baseline of the uterus and ovaries and documents the antral follicle count.
The scan is performed transvaginally because this approach provides the resolution needed to assess small ovarian follicles and measure the endometrium accurately. Both ovaries are identified and measured, and the antral follicles visible in each ovary are counted.
The report can be shared with your IVF clinic. Your fertility specialist remains responsible for interpreting the scan together with AMH, age, previous treatment history, blood tests and the planned stimulation protocol.
What is included in the pre-IVF baseline scan?
The examination is structured around the measurements commonly required before assisted conception while also documenting relevant pelvic findings that may affect interpretation.
Uterus
Assessment of uterine position, dimensions, contour and myometrial appearance, including visible fibroids or other structural abnormalities where present.
Endometrial thickness
Measurement of the endometrium with documentation of its baseline ultrasound appearance for the stage of the cycle.
Right ovary
Right ovarian dimensions and baseline appearance, with antral follicles counted individually where visible.
Left ovary
Left ovarian dimensions and baseline appearance, with antral follicles counted individually where visible.
Total antral follicle count
Right and left AFC are documented separately and combined to provide the total baseline antral follicle count.
Adnexa & baseline findings
Relevant ovarian cysts, adnexal findings, pelvic free fluid or other visible baseline abnormalities are documented where present.
Key structures assessed during your baseline fertility scan
Simplified anatomy illustration for patient education.
A key ultrasound marker of expected ovarian response
Antral follicle count is one of the recognised ultrasound measures used alongside the wider fertility assessment to estimate likely ovarian response to stimulation.
How AFC is reported
The Sonographer identifies both ovaries and counts the small antral follicles visible within each ovary. The right and left counts are recorded separately, together with the combined total.
Why a transvaginal scan is required for this service
The ultrasound probe is positioned close to the uterus and ovaries, providing the high-resolution views needed for baseline IVF measurements and antral follicle counting.
High-resolution transvaginal imaging
A slim covered ultrasound transducer is inserted gently into the vagina after the examination has been explained and you have given explicit consent.
Consent-led intimate examination
Transvaginal ultrasound is an intimate examination. It is only performed after clear explanation and explicit consent, with privacy and dignity maintained throughout.
When a pre-IVF baseline ultrasound may be requested
This scan is intended for patients preparing for IVF, ICSI or another assisted-conception pathway who need a baseline pelvic ultrasound before treatment or stimulation.
Before an IVF cycle
Baseline uterine, endometrial and ovarian measurements before planned ovarian stimulation.
Before ICSI treatment
Baseline imaging requested by a fertility provider as part of treatment planning before stimulation.
Ovarian response planning
AFC can contribute to assessment of expected ovarian response when interpreted by the fertility team with the wider clinical picture.
Endometrial baseline
Measurement of endometrial thickness and baseline appearance before treatment begins.
Check for ovarian cysts
Baseline ovarian imaging may identify cysts or other findings that should be documented before treatment.
External fertility clinic request
Suitable for patients whose IVF provider has requested baseline pelvic measurements and a written ultrasound report.
The baseline scan informs treatment planning — it does not determine IVF outcome
The ultrasound provides structural and ovarian-reserve information for your fertility team. Treatment decisions remain with the clinician managing your IVF or assisted-conception cycle.
AFC predicts response, not pregnancy
AFC is used to help estimate likely ovarian response to stimulation. It should not be interpreted as a stand-alone prediction of pregnancy, embryo quality or live birth.
Ultrasound does not replace AMH or fertility consultation
Your fertility clinic may use AMH, age, previous cycle response, medical history and other investigations alongside the ultrasound findings.
Relevant findings may require further review
Fibroids, ovarian cysts, suspected adenomyosis, endometrial abnormalities or adnexal findings may require review by your fertility clinician, gynaecologist or additional imaging.
What happens during your pre-IVF baseline scan?
A structured fertility-focused examination with measurements documented clearly for onward use by your fertility provider.
Clinical history
Your IVF plan, cycle information, previous fertility treatment and relevant pelvic history are reviewed before scanning.
Consent & TVUS
The transvaginal examination is explained and explicit consent is obtained before the scan begins.
Baseline measurements
The uterus, endometrium and both ovaries are assessed, with ovarian dimensions and AFC documented.
Report for your clinic
The main findings are explained and a same-day written report records the baseline measurements and relevant findings.
Preparing for your baseline IVF ultrasound
Your fertility clinic may specify when in your cycle the baseline scan should be performed. Follow their protocol where one has been provided.
Follow your fertility clinic’s timing
Baseline scans are commonly requested before stimulation and may be scheduled early in the cycle. Your treating clinic’s instructions take priority.
Empty bladder for TVUS
The transvaginal examination is normally performed with an empty bladder. You will be given privacy to prepare before the scan.
Bring your clinic request or protocol
If your fertility provider has requested specific measurements, follicle-size recording or reporting terminology, bring those instructions to your appointment.
Need monitoring before or after your IVF baseline scan?
Different stages of fertility treatment require different ultrasound protocols. Choose the service that matches the request from your fertility team.
Follicular Tracking
Repeat transvaginal ultrasound to monitor developing follicles and endometrial thickness through the cycle.
Female Pelvic Ultrasound
A broader gynaecological pelvic ultrasound where the main question is structural pelvic assessment rather than an IVF baseline protocol.
IVF Pregnancy Scan
Ultrasound assessment after a positive pregnancy test following IVF, according to the appropriate gestational timing.
